Areas Reviewed:
A monitoring inspection was conducted on 07/18/2022. There were 42 children present and 6 staff. The inspector conducted a tour of the facility, reviewed records and made observations of the program. A sample of 4 children and 3 staff records were reviewed. Children were observed on the playground, doing an art activity and infants were observed in cribs and on the floor with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a safe condition.
Evidence:
1. There was peeling paint in the following areas: door separating lobby from the classrooms, the infant room doorway, The wall at the "bird watching station" in the 4 year-old room, and the daycare hallway.
2. In the 4 year-old room a cot was propped up, sideways along a door. A sign behind it stated there was peeling paint in this area and not to touch.
3. In the girl's bathroom, one of the toilet seats was cracked and the clear coating on 2 toilet seats was peeling.
4. In the boys bathroom the clear coating on one of the toilet seats was peeling. The bathroom had a strong urine smell. The Director confirmed she was aware.
5. The shed in the outside play area had rotted wood at the front base of the structure.
6. There were 2 plastic sand buckets on the play area that were cracked, sharp and had jagged edges.

Plan of Correction: New work orders have been entered for all Facility Maintenance issues. These are all on separate tickets so the ones that are easiest to correct can be addressed immediately and longer time items will be addressed separately.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place.
Evidence:
In the 3 year-old room there was an unlocked cabinet containing a spray bottle of glass cleaner and a disinfectant spray.

Plan of Correction: Chemicals will be locked and staff will be reminded to keep all chemical cabinets locked.

Standard #: 8VAC20-780-320-B
Description: Based on observation the center did not ensure restrooms had all required items.
Evidence:
On of the 2 restrooms for the children did not have paper towels.

Plan of Correction: The center was short staffed and the director was in a classroom and had not been able to refill bathroom supplies. The director will remind staff that supplies should be refilled as soon as it runs out.

Standard #: 8VAC20-780-330-B
Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
The playground structure on the side of the building did not have at least 6 inches of resilient surfacing in the fall zones. Grass and weeds were growing above the surfacing in the fall zone areas.

Plan of Correction: Resilient surfacing has been ordered and center is waiting for delivery.
