Description: Based upon review of staff records and staff interview, the facility has not ensured that they have obtained the results of a child protective services (CPS) central registry finding within 30 days of employment.
Evidence:
1. The record provided for staff 2 indicated a hire date of 5/2/2022. The CPS central registry request was returned to the facility for corrections on 5/13/2022. There was no documentation indicating additional follow-up by the facility as the findings had not yet been returned. Staff 2 was on duty with the children during the inspection.
2. Staff 1 acknowledged that the CPS central registry request had been returned for information but that there had been no additional follow-up when the finding was not received.

Plan of Correction: The facility responded with the following:
Documentation was sent out in June. Check was cashed July 1st. We are still waiting results.

Standard #: 8VAC20-780-70
Description: Based upon review of staff records and staff interview, the facility has not ensured that each staff record includes all required documentation.
Evidence:
1. The record provided for staff 2 included documentation that only one reference check as to character, reputation and competency was checked prior to employ. The one reference check in the file was not dated.
2. The records provided for staff 3 and staff 4 indicated that they are program leaders, however neither record includes any documentation to demonstrate how the staff are qualified for the program leader positions.
3. Staff 1 verified that the record for staff 2 had only one documented reference check and that the records for staff 3 and 4 did not include documentation of qualifications.

Plan of Correction: the facility responded with the following:
1. The references have been placed in employee's file.
2. Additional training is already in place for daycare staff.

Description: Based upon observation and staff interview, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by the children.
Evidence:
1. There was a container of bleach with a warning label in an unlocked cabinet under the sink in the facility hallway.
2. Staff 1 acknowledged the bleach in the unlocked cabinet under the sink.
3. There was a container of antibacterial hand soap with a warning label on the sink in the hallway bathroom. There was another container of antibacterial hand soap on the hand washing sink in the toddler/preschool classroom.

Plan of Correction: The facility responded with the following:
Hand soap containers have been changed out.

Standard #: 8VAC20-780-340-F
Description: Based upon observation, the facility has not ensured that children under 10 years of age are always within actual sight and sound supervision of staff.
Evidence:
Staff 2 was observed standing in the doorway to the toddler/preschool classroom at the far end of the main hallway while allowing child 1, four years old, to walk down to the far end of the hallway to use the bathroom. Child 1 was not in sight and sound supervision of staff 2 while using the bathroom.

Plan of Correction: The facility responded with the following:
In the process of hiring more staff and volunteers to assist the daycare.

Standard #: 8VAC20-780-350-B-2
Description: Based upon observation, the facility has not ensured that the ratio of staff to children for children ages 16 months - 24 months is 1 staff person for every 5 children.
Evidence:
From 10:56 AM until 11:15 AM, staff 2 was alone in a classroom of six children from ages 20 months through 4 years while staff 5 went to the church kitchen to prepare lunch.

Plan of Correction: The facility responded with the following:
In the process of hiring new staff to accommodate the number of children in calssrooms.

Standard #: 8VAC20-780-550-P
Description: Based upon review of injury records, the facility has not ensured that they maintain a written record child children's injuries that included all required documentation.
Evidence:
A written injury record dated 6/30/22 for child 2 did not include either two staff signatures or a staff signature and a parent signature.

Standard #: 8VAC20-780-570-E
Description: Based upon observation and staff interview, the facility has not ensured that prepared infant formula is dated and labeled with the child's name.
Evidence:
1. There was a prepared infant bottle on a table next to staff 3 in the infant room. The bottle was not labeled with a child's name or the date.
2. Prepared infant formula in the refrigerator in the infant room was not dated.
3. Staff 5 acknowledged that not all prepared bottles had names and none of the bottles were dated.
