Comments:
The licensing Inspector initiated and completed a renewal inspection on 7/18/2022. The inspector was on site from 9:20am until 11:30am.

There were 67 children present under the supervision of 10 staff. The ages of the children ranged from 5 months to 12 years old. The children were observed during group time and outside play. Six children?s records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to have area outside maintained in a clean, safe and operable condition.

Evidence:

1. On outdoor School Age playground, the fence has come unattached to the pole which keeps it in place. This is causing the fence to buckle backward. Also, the metal piece that ties the fence to the pole is exposed and sticking out. This causes an impalement hazard to the children.


2. Metal pins that are used to keep the plastic barrier in place are coming approximately ? inch up from original placement. This poses a tripping hazard for children.

Plan of Correction: Maintenance will be out to fix all playground issues.

Standard #: 8VAC20-780-510-F
Description: Based on review of medication, the center failed to obtain written permission from a parent or guardian to administer medication.

Evidence: Child #3 had no written authorization to administer a prescription medication observed at the center.

Plan of Correction: Medication will be removed form site until site has authorization paperwork.

Standard #: 8VAC20-780-510-G
Description: Based on review of medication, the center failed to ensure medication was labeled with child?s name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: Prescription Medication for Child #2 was observed in a plastic bag, with an index labeled with the child?s name on in. There was no label on the medication to provide information on what the medication was, dosage amounts or times to be given.

Plan of Correction: Medication will be removed from the site until parent has proper label for it.
