A monitoring inspection was conducted on 07/20/2022. There were 65 children present and 15 staff. A tour of the facility, records were reviewed and observations of the program were made. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-260-A Description: Based on a record review the center did not ensure the facility had an annual fire inspection.
Evidence:
The last documented fire inspection provided by the assistant director was dated 06/24/2021.Plan of Correction: The City was contacted and stated they are behind which is why they didn?t come out for the annual. They are looking to fit us in within the next 30 days for the inspector to come out. Going forward we will make sure we have it in writing if our inspections aren?t able to be completed by the city on time. Management will monitor periodically to make sure all inspections are complete and compliant.
Standard #: 8VAC20-780-270-A Description: Based on observation the center did not ensure all area and equipment of the center were maintained in safe condition.
Evidence:
1. The classroom door frames throughout the building had chipped paint.
2. In the school-age room there was a table with the laminate top chipped off.Plan of Correction: This will be corrected by 8/22/2022. The safety captain and teachers will do classroom and building walk throughs daily. Any deficiencies will be reported to management within 24 hours. Management will do periodic walk throughs to ensure our building is maintained in a safe condition.
Standard #: 8VAC20-780-500-A Description: Based on observation the center did not ensure all handwashing requirements were met.
Evidence:
A staff in the infant room was observed changing a child's diaper. After the change, the staff placed did not wash the hands of the baby or her own, before proceeding to gather clean diapers and clean the changing table.Plan of Correction: All staff in diapering classrooms will be retrained by 7/25/2022. Our safety captain and management team will do unannounced observations to ensure compliance.
