Technical Assistance:

A review of the background checks and documentation on the new Executive Director of Operations was conducted. Discussion of Out-of-State background checks.

Comments:

This monitoring inspection was initiated by licensing staff and concluded on 07/21/2022, between the hours of 9 am to 3 pm. There were 90 children present, ranging from infant to school-age, with 14 staff supervising across 9 separate classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 10 children records, and 10 staff records were reviewed. Information gathered during the inspection determined that there were (5) non-compliances with the applicable standards, and code. Standard #:	22.1-289.035-B-4
Description:	Based on review, 4 of 10 staff files indicated that the child day center (CDC) did not obtain a copy of the results of a criminal history record information check (CRC), a sex offender registry check (SOR), and a search of the child abuse and neglect registry (CPS) or equivalent registry from any state in which the individual has resided in the preceding five years before reporting to the CDC. Evidence - 1. On the date of inspection 07/21/2022, the following staff were missing the following documentation: Staff #7 (start date - 07/18/22) a CPS check from the State of Michigan; Staff #8 (start date - 07/05/2022) a CPS check, a SOR check, and a CRC from the State of Washington; Staff #9 (start date - 07/05/2022) a CPS check, a SOR check, and a CRC from the states of Pennsylvania, Connecticut, South Dakota, Massachusetts, and a CPS check from the State of Tennessee; and Staff #10 (start date - 07/20/2022) a CPS check, a SOR check, and a CRC from the Commonwealth of Pennsylvania, and a CPS check from the states of Florida, and Connecticut. 2. All four staff were observed on site on the date of inspection.

Standard #:	8VAC20-780-140-A
Description:	Based on review, 1 of 10 children files indicated that each child did not have a physical examination (by or under the direction of a physician) on file at the center within 30 days of their start date. Evidence - 1. On the date of inspection (07/21/2022), documentation of a physical was not available for review for Child #2. 2. The start date for Child #2 was listed as - 03/16/2021. Child #2 was in attendance on the date of inspection.
Plan of Correction:	All missing documentation will be obtained.
Standard #:	8VAC20-780-60-A
Description:	Based on review, 3 of 10 children files indicated that not all of the required information / documentation was contained within each child's record. Evidence - 1. On the date of inspection (07/22/2022), the health information (both the immunization page and documentation of a physical) was not on file for the following children: Child #2, Child #6, and Child #7. 2. This was confirmed on the date of inspection by the Center Director.
Plan of Correction:	All missing documentation will be obtained.
Standard #:	8VAC20-780-70
Description:	Based review, 6 of 10 staff files indicated that not all of the required information / documentation was on file within each staff record. Evidence - 1. On the date of inspection (07/21/2022), the following staff were without at least 2 references on file: Staff #4, Staff #6, Staff #7, Staff #8, Staff #9, and Staff #10. 2. All staff were hired after March 1, 1996.
Plan of Correction:	All missing documentation will be obtained.
Standard #:	8VAC20-780-530-A
Description:	Based on observation, there was not at least one staff in each classroom or area where children were present that was certified in both CPR and First Aid. Evidence - 1. On the date of inspection 07/22/2022 at approximately 10 am, there was one staff person with 3 children in the Toddler Room. 2. This person, Staff #8, was not currently certified in both CPR and First Aid.
Plan of Correction:	A CPR and First Aid course will be given the night of 07/22/2022. This staff person and several others are signed up to receive this certification.
