Technical Assistance:
During the inspection and exit interview, the following were discussed
? Over the counter skin products
? Crib mattress use
? Allergy care plan

Comments:
An unannounced monitoring inspection was conducted today at 9:00am-12:30pm. There were 71 children ranging in age from 4 months old to 10 years old supervised by 13 staff. The physical plant, programming, menus, 5 staff records, 7 children?s records, 9 medication, emergency
drills, policies and emergency supplies were inspected. Children were observed napping, eating snack, diapering, getting ready for outdoor play, and playing on the playground. There was an adequate number of staff trained in CPR/First aid at the time of inspection. Areas of noncompliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on review of documentation and interview with the staff, the center did not maintain a current and dated written list of children?s allergies, sensitivities, and dietary restrictions that was accessible to all staff in each group or area.

Evidence: 1)In the infant classroom, there was no dated allergy list posted for the staff.

Plan of Correction: Allergy lists were updated and posted in classrooms. Classrooms with no allergies have a posting that states: No allergies in Classroom.

Standard #: 8VAC20-780-80-A
Description: Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.

Evidence: 1)During inspection in the school age classroom, there were 18 children present,
but the written record reflected there were 17 children present.

Plan of Correction: Staff will be retrained in classroom attendance procedures as well as name to face checks to ensure accurate daily attendance.

Standard #: 8VAC20-780-90--A
Description: Based on review of five child records, the center did not obtain parent authorization for
emergency medical care should an emergency occur when the parent cannot be located
immediately.

Evidence: The record for Child #5 did not contain parent authorization for emergency medical care in case of an emergency when the parent cannot be reached.

Plan of Correction: Forms were signed by parent at pick up.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence: 1)The lower part of the wall next to the block area in the school age classroom, has large areas of paint and drywall chipping from the wall. 2)On the Fours/School Age playground and on the Twos/Threes playground, there are 4 large screens that are within reach of children and are torn/shredded. 3) On the Twos/Threes playground, there is a large sharp rock protruding from the mulch area on the hill in the back which could pose a tripping hazard or snag to the skin. 4)On the Toddler playground, the resilient surfacing next to the stairs that lead to the slide, has some cracking, a large hump, and a void under the resilient surfacing that could cause a trip hazard.

Plan of Correction: A maintenance company has been contacted to repair playground surface and paint classroom walls. Rocks will be removed from playgrounds. Replacement screens have been ordered to be installed on windows.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a
locked place using a safe locking method that prevents access by children.

Evidence: 1)During inspection in the twos classroom, there was a bottle of sanitizer that was left out on the counter in an unlocked and accessible area next to the diapering station. 2)Also in the twos classroom, there were sanitizer and disinfectant that was in an unlocked cabinet and accessible to children. 3)In a utility closet off the hallway, where a sign was posted that read ?Please keep door locked at all times?, the door was unlocked and contained 2 multipurpose cleaners stored on the floor, a bottle of caulk, and 2 spray bottles
of unlabeled cleaners that were accessible to children.

Plan of Correction: Staff will be retrained in proper chemical storage. Management will assist and observe to ensure hazardous substances are stored in a locked cabinet. Utility closet door will be closed and locked at all times. Management will conduct periodic checks throughout the day to ensure that the utility door is locked
throughout the day.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the center did not ensure that staff wash their hands with soap and running water after a diaper change and after contact with body fluids.

Evidence: In the twos classroom, both staff members did not wash their hands after changing child diapers and before touching other surfaces and children in the classroom.

Plan of Correction: Staff will be retrained in proper handwashing procedures and management will assist and observe to ensure protocols are followed.

Standard #: 8VAC20-780-520-B
Description: Based on observation, the center did not ensure that sunscreen was labeled with the child?s name and that it was inaccessible to children.

Evidence: 1)In the twos classroom, a basket of
sunscreen was stored in a lower cabinet that was left unlocked and accessible to the children. 2)In the threes classroom, there was sunscreen in a basket that was not labeled with the child?s name as required.

Plan of Correction: Sunscreen was sent home and new sunscreens were requested. Staff went
through sunscreens to ensure sunscreen was labeled with student's names.
All sunscreen was moved to locked cabinets and staff will be retrained in
sunscreen storage policies.

Standard #: 8VAC20-780-550-P
Description: Based on review of documentation, the center did not ensure that the written record of children?s injuries contained all required information.

Evidence: During inspection, accident reports dated 6/8/22 for Child #1, 6/8/22 for Child #2,
5/31/22 for Child #3, and 7/12/22 for Child #4 were missing the future action to prevent
recurrence of the injury.

Plan of Correction: Staff will be retrained in accident report writing procedures. Going forward accident reports will a reflect plan of action for prevention of recurrence of injuries.
