Comments:

An unannounced monitoring inspection was conducted today between the hours of 10:30am-12:45 pm. There were a total of 92 children with 12 staff members present. Children were observed during water play, handwashing, diapering, toy play, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #:	8VAC20-780-130-E
Description:	Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. Child #5's record (Start date-1-20-22) did not contain additional immunizations once every six months. The last immunization record was dated 12/16/21.
Plan of Correction:	The parents were contacted during the time of the inspection and provided a copy of additional immunizations.
Standard #:	8VAC20-780-280-B
Description:	Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: In the three's classroom, five bottles of cleaner and sanitizer was located in a closet that was not locked.
Plan of Correction:	The closet will be locked.
Standard #:	8VAC20-780-280-D
Description:	Based on observation, cleaning and sanitizing materials were stored in areas with food. Evidence: 1. In the Infant B classroom, cereal and snacks were stored on a bottom shelf with four bottles of cleaner and sanitizer.
Plan of Correction:	The hazardous substances were removed from the shelf with the food at the time of the inspection. Food will not be stored with hazardous substances.

Description:	Based on observation, cleaning materials (e.g., detergents, sanitizers and polishes) and insecticides/pesticides were not stored in areas physically separate from each other. Evidence: 1. In the laundry room, a bottle of bleach, powdered laundry detergent, cleaning materials, and paint were stored together.
Plan of Correction:	The items will be stored separately.

Description:	Based on observation, the center did not have toilets that are flushable. Evidence: 1. One of the three toilets located between the 2's classroom and the Toddler classroom did not flush.
Plan of Correction:	I will check the toilets and fix it.

Description:	Based on record review, staff did not request at least annually parent confirmation that the required information in the child's record is up to date. Evidence: 1. Child #2's record (Start date-1/4/21) did not contain documentation of an annual update,
Plan of Correction:	The parents will review the child's record and document the record.
Standard #:	8VAC20-780-500-A
Description:	Based on observation, children's hands were not washed with soap and running water after diapering. Evidence: 1. In the toddler room, a child's hands were not washed after diapering. 2. Staff #3 did not wash their hands with soap and water after diapering.
Plan of Correction:	We will retrain staff on handwashing.

Description:	Based on observation, the diapering surface was not cleaned with soap and water and sanitized after each use. Evidence: 1. In the Toddler room, the diapering table was not cleaned and sanitized after each use.
Plan of Correction:	We will retrain staff on cleaning and sanitizing the diapering table after each use.

Description:	Based on review of documentation and observation, diaper ointment or creams used were not labeled with the child's name and did not have written parent authorization. Evidence: 1. In the Infant B classroom, 1 of 5 diaper creams were not labeled and did not have parent authorization. 2. In the two's classroom, 8 diaper creams did not have parent authorization and 5 diaper creams were not labeled with the child's name. 3. In the Toddler classroom, 1 of 8 diaper creams were not labeled.
Plan of Correction:	Parent authorizations will be obtained for the diaper creams and the diaper creams will be labeled with the child's name.

Description:	Based on review of documentation, the center did not maintain a written record of children's serious and minor injuries with required information. Evidence: 1. In the Infant B classroom, 6 of 6 accident reports were missing the actual time that parents were notified of the accident.
Plan of Correction:	All accident reports will have an actual time for the time of notification.
