A monitoring inspection was initiated on 8/1/2022 and concluded on 8/1/2022. There were a total of 46 children with 11 staff members present. Children were observed engaged in group activities, making birthday card, blocks and outdoor play time. A total of 3 child records and 3 staff records were reviewed. Fire drills and carbon monoxide detector were observed. The phone number for central registry was given to follow up on the 2 missing results which were mailed over 2 months ago. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-4 Description: Based on a review of staff records, the center has not obtained a copy of the results of a criminal history record information check from any state in which the individual has resided in the preceding five years.
Evidence: Staff #1, date of hire 9/8/2021 has documented on the sworn statement they reside in Washington, D.C. There is no documentation that a criminal history results have been obtained.Plan of Correction: Administration will have the staff obtain a criminal history from Washington DC. This result needs to be completed in person and the next available appointment is 9/30/2022. The results will be filed after the 9/30/2022 appointment.
Standard #: 8VAC20-780-510-P Description: Based on medication review, when an authorization for medication expires, the parent has not been notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.
Evidence: Child #1 has an emergency medication, the authorization for this medication expired 6/23/2022 and the parent has not been notified.Plan of Correction: Administration will ask the parent today to have a new form completed by the physician.
