Comments:
An unannounced on-site monitoring inspection was conducted on 7/26/2022. There were 34 children, ages 8 months-5 years, and 10 staff members present during the inspection. The inspector observed the following: free choice lunch, large motor and nap. A total of 5 children?s records and 5 staff records, were reviewed. The inspector arrived at 12:00 and departed the program at 2:30

Violations:
Standard #: 8VAC20-780-40-K
Description: Based on review of policies and procedures, the center failed to ensure all required components were established in the prevention of shaken baby, or abusive head trauma, including coping with crying babies, safe sleep practices and sudden infant death syndrome awareness.

Evidence:
1.The procedure that was established did not include Sudden Infant Death Syndrome and safe sleep practices.

Plan of Correction: The director will add the procedure to the handbook.

Standard #: 8VAC20-780-280-B
Description: Based on observation of the preschool room, the center failed to ensure all chemicals were locked.

Evidence:
1.The inspector observed two containers of chemicals sitting on a shelf in the laundry area. The children have access to the area. The chemicals are stored on a high shelf.

Plan of Correction: The staff locked the chemicals during the inspection and will ensure that they are locked when not in use.

Standard #: 8VAC20-780-350-I
Description: Based on observation, the center failed to ensure that all rest time ratio were met.

Evidence:
1.OT and YT classrooms were combined for rest time on the day of the inspection. The ages of children were 12 months to 2 years. The center was doubling the ratio during rest time. The standard does not allow for children under 16 months of age to have a doubled ratio during rest time.

Plan of Correction: The director will rearrange rest time to ensure that all ratios are met.
