Technical Assistance:
Technical assistance was provided in the following areas of the standards: Background checks, staff records, required training, Program Leader qualifications, supervision, emergency drills, attendance, CPR/First aid certification, carbon monoxide detectors, infant feeding, and allergies.

Comments:
An unannounced monitoring inspection was conducted on 8/2/22 from 9:30am - 11:40am. During the inspection there were 26 children ages 12 months old through five years old in care with 8 staff. Children were observed participating in various activities in the classrooms, playing outside, and eating snack. Records were reviewed for five children and six staff during the inspection. Medication, emergency procedures, emergency supplies and transportation procedures were also reviewed during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed during the exit interview.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of five staff records, it was determined that the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and shall have been completed in the last 30 calendar days.

Evidence:
1. The record for staff #1 (date of hire 3/7/22) contained documentation of a negative tuberculosis screening that was dated 9/9/21.
2. Staff #7 reviewed the record for staff #1, and confirmed that the negative tuberculosis screening for staff 31 had been completed more than 30 days prior to employment.

Plan of Correction: The facility responded: Staff #1 will complete a new TB screening. All new staff will have a Tb screening completed within 30 days prior to employment.

Standard #: 8VAC20-780-240-A
Description: Based on a review of six staff records, it was determined that the licensee did not ensure that all new employees complete the Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Evidence:
1. The record for staff #1 (date of hire 3/7/22) did not contact documentation to demonstrate staff #1 had completed the Virginia Department of Education-sponsored orientation course.
2. Staff #7 confirmed that staff #1 did not complete the Virginia Department of Education-sponsored orientation course within 90 days of employment.

Plan of Correction: The facility responded: Staff #1 is currently not working, but we will have her complete the Virginia Department of Education-sponsored orientation course. All new staff will complete the Virginia Department of Education-sponsored orientation course within 90 days of employment.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interviews, it was determined that the licensee did not ensure that all areas and equipment of the center shall be maintained in a safe and operable condition.

Evidence:
1. The wooden storage shed located on the playground has areas where the paint is peeling.
2. Staff #7 confirmed the storage shed was not in a safe and operable condition.

Plan of Correction: The facility responded: We will have the storage shed painted.

Standard #: 8VAC20-780-570-I
Description: Based on observation and interview, it was determined that the licensee did not ensure that the facility shall maintain a one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care.

Evidence:
1. There was not a 24 hour supply of formula, bottles, and nipples at the facility..
2. Staff #7 confirmed there was not a 24 hour supply of formula, bottles, and nipples at the facility..

Plan of Correction: The facility responded: We will purchase 24 hour supply of bottles, nipples, and formula.
