Technical Assistance:
1. The inspector agreed to re-send the Director a quick reference guide to ?new? regulations.
2. Although the license to operate has expired, a renewal application has been submitted by the licensee, and the child day center is considered to be operating legally. Questions about this may be directed to pamela.sneed@doe.virginia.gov

4. The JCCNV Board Member background checks are pending completion due to technical issues and subsequent delays with State?s processing database

Comments:
An unannounced renewal inspection was conducted at 10:50am. 20 children + 5 direct-care staff + 2 managers were on-site today. Ratios and supervision were in compliance. Children were observed playing with toys, having educational water play, and having outdoor active-play on the playground. Staff were actively engaged with the children and activities in each group. Classrooms were found to be clean and well-supplied with toys, materials, and furnishings. The ?new? outdoor playground offers a multitude of creative and child-driven educational play opportunities. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Violations:
Standard #: 22.1-289.035-B-4
Description: Based on records reviewed, it was determined that 1 of 3 staff records did not include required background checks. Evidence: Staff #11 - An out-of-state Sex Offender Registry check was not completed prior to the 1st day of employment, and to-date has not been completed. Date of hire was 6/20/22.

Plan of Correction: Both checks were completed on the day of the inspection

Standard #: 8VAC20-780-160-C
Description: Based on records reviewed, it was determined that 1 of 3 staff records did not include required information. Evidence: The record for Staff #4 did not include the results of a TB screening within the last 2 years. The previous TB test expired 7/9/22.

Plan of Correction: Said teacher is on vacation TB test scheduled for 1st week in Aug - when she returns

Standard #: 8VAC20-780-260-A
Description: Based on interview conducted, it was determined that an annual fire inspection has not been completed for the facility. Evidence: The program director reported that she submitted a request for the fire inspection to upper management, but she is unsure if it has been conducted an does not have the subsequent fire inspection report.

Plan of Correction: Get the inspection report from AFD. We have the receipt from 11/21 so its done-trying to get the paperwork ASAP. Calls to AFD

Update: The CDC submitted a fire inspection report dated 8/2/22.

Standard #: 8VAC20-780-330-B
Description: Based on observations made, it was determined that resilient surfacing was lacking in fall zones on the playground. Evidence: The fall zones surround the slide structure and the climbing tree structure did not have a minimum of 6" of wood mulch. Some of these areas had a scattering of mulch over the dirt surface.

Plan of Correction: The program director has contacted 3 contractors for an estimate of cost to install wood mulch, and is waiting on those cost estimates. Until the mulch is replenished in those fall zones, children will not be permitted on those climbing structures.

Update: Contract signed-waiting for scheduler to arrange delivery. Scheduled for 8/16/22.
