Comments:
An unannounced Monitoring Inspection was conducted today from TIME. There were 21 children (Infants to School-Age) directly supervised by 8 staff. The physical plant, 2 staff records, 4 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside, participating in group and individual activities, napping and eating. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Description: Based on review, the facility did not ensure that children under 2 years of age were providing documentation of additional immunizations every 6 months.
Evidence: Reviewed 3 children's records and found that Child 1 and Child 2 did not have documentation of updated immunizations every 6 months.

Standard #: 8VAC20-780-60-A
Description: Based on review, the facility did not maintain and keep at the center a separate record for each child enrolled which shall contain the required information.
Evidence:
1) identified through staff interviews that Child 4 did not yet have a record maintained at the facility, but is currently in care.
2) reviewed 3 children's records and found that Child 3 did not have documentation of the parent's annual review of the child's emergency contact information.

Description: Based on review, the facility did not ensure that staff records were kept for each staff person and contained the required minimum information.
Evidence:
1) reviewed 2 staff records and found that staff 1 and 2 did not have documentation of 2 references completed prior to employment.
2) reviewed 2 staff records and found that staff 1 did not have documentation of completed background checks. The owner stated that the background checks were completed, but had not been filed.

Description: Based on observation, the facility did not ensure that hazardous substances were kept in a locked place, using a safe locking method that prevents access by children.
Evidence: observed a large container of hand sanitizer, marked keep out of reach of children, accessible to children on a shelf near the front entrance of the school, where children's coats and personal belongings are kept.

Standard #: 8VAC20-780-340-A
Description: Based on observation, it was determined that the staff did not ensure the care, protection and guidance of children.
Evidence: Upon arrival at the facility the licensing inspector (LI) observed two children in the 2-3 year old class were observed riding children's push scooters in the center's driveway, used by the facility and 3 other businesses to enter the parking lot. The LI was driving, and had to slow down, and then stop to avoid coming into contact with the children. The children were approximately 8-10 feet from the entrance ramp that led to the main entrance to the facility.
A staff member was standing at the top of the ramp, approximately 20-40 feet from the children, calling to the children. No staff were close enough to intervene if the children were to come into contact with an incoming vehicle, or to prevent the children from riding into the street at the end of the driveway.

Evidence: observed in the infant classroom, a child, approximately 5 months old, sleeping in a crib, cradled on a round pillow.
