Standard #: 8VAC20-780-40-E
Description: Based on observation, interview and a record review the center did not ensure the operational responsibilities were maintained in compliance with the standards and the center's own policies and procedures.
Evidence:
1. Staff in the 3 year-old room did not use the face-to-name document required by the center to ensure ratios and supervision are maintained.

2. The inspector asked the staff member how many she had in her care and the staff replied, "14 children." The inspector counted the children in the room to discover there were actually 21 children and the room was not in ratio with 21 napping children and 1 staff present.

3. The staff member checked the face-to-name paperwork and it showed there were 19 present.
The staff stated she did not know who the 2 children were that were unaccounted for.

4. The director confirmed face-to-name sheets are to be done each transition that occurs and should've been done prior to nap.

Plan of Correction: All staff will be retrained on face to name supervision and expectations on or before 8/8/22. Management will be responsible for reviewing and validating all face to name sheets daily to prevent re-occurrence.

Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
In the pre-k classroom there was a children's red chair that was torn with the inside stuffing exposed.

Plan of Correction: All torn and ripped soft element pieces will be removed from the school on or before 8/12/22. Our safety captain will check weekly to ensure we don?t have torn or broken equipment in the classrooms.

Standard #: 8VAC20-780-350-I
Description: Based on observation and interview, the center did not ensure the required ratio during rest periods.
Evidence:
The ratio for 3 year-old children during nap is 1 staff to every 20 children. There were 21 three year-old children observed napping on cots with 1 staff present.

Plan of Correction: At no time will staff be allowed to run out of ratio. Management will check nap rooms daily to ensure compliance. Naptime ratios have been reviewed with all staff as of 8/8/22.
