Comments:

An unannounced monitoring inspection was conducted on August 1, 2022 from approximately 10:45 am to 1:26 pm. There were 72 children in attendance and a total of 13 staff present. 7 staff records and 7 children?s records were reviewed. All areas of the center were observed including classrooms, hallways, kitchen, and bathrooms. The children were observed listening to stories, singing, playing outside, coloring, and getting diapers changed, while the licensing inspector was on the premises. Emergency drills and fire inspections were in compliance. The most recent Health Inspection was on 04/19/22. The most recent Fire Inspection was on 04/07/22. The most recent fire drill was on 07/29/22. Shelter in Place Drills were conducted twice with the most recent on 05/27/22. The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted by each phone. Violations were cited as a result of this inspection. Standard #:	8VAC20-770-60-B
Description:	Based on a review of documentation, the center did not obtain a fully completed sworn statement prior to employment. Evidence: Staff #3?s record, date of employment 05/16/2022, contained a completed Sworn Statement that did not contain a date.
Plan of Correction:	Per the center "upon return to work, staff #3 will fill out a new Sworn Statement with date and note in file. Moving forward all new hire paperwork will be double checked by management."
Standard #:	8VAC20-770-60-C-2
Description:	Based on a review of documentation. the center did not ensure that all employees had a CPS background check mailed within 7 days of employment. Evidence: 1) Staff #7, DOH 06/29/22 did not have evidence that the CPS background check was mailed. Evidence was requested to be emailed to the Inspector. As of the date of this report the evidence had not been sent.
Plan of Correction:	Per the center "correspondence sent to DSS to check status and receipt date. Moving forward all CPS background checks will be mailed last day of training with copy of receipt kept."

Description:	Based on a review of documentation, the center did not ensure that staff completed the Virginia Department of Education (VDOE) sponsored orientation course within 90 days of employment. Evidence: Staff #4, DOH 11/21/21 did not have evidence of taking the VDOE sponsored orientation course. Evidence was requested to be emailed to the inspector. As of the date of this report the evidence had not been sent.
Plan of Correction:	Per the center "will review and ensure that all staff complete 10 hour and annual 3 hour as required."

Description:	Based on a review of documentation, the center did not document monthly practice evacuation drills. Evidence: The drill log did not document a drill during the month of June 2022.
Plan of Correction:	Per the center "will conduct two drills in the month of August."
Standard #:	8VAC20-780-550-P
Description:	Based on a review of documentation, the center did not maintain a written record of the day and time when parents are notified of children?s injuries. Evidence: 3 out of 3 injury reports viewed did not have a day or time indicating when the parent was notified of the injury.
Plan of Correction:	Per the center "staff and management will review all incident/accident forms and ensure they are fully completed."
