Comments:
An investigation was conducted at the facility on June 30, 2022 and July 6, 2022 after a complaint was received at the Fairfax licensing office on June 29, 2022. The allegation regarding supervision and ratios were reviewed. The center has been provided with the findings of the investigation. There were also observed in the play area violations that were not reported but found. Please call with questions 703 479-4679.

Description: Based on observation of the outside play area, it was determined the playground surfacing has not been designed to protect children from hazards.
Evidence:
1. The playground uses pea gravel for resilient surfacing. The pea gravel is not adequate in depth throughout the playground which results in exposed tree roots that span around the playground. The roots pose tripping hazards throughout the play yard.

Plan of Correction: We have a shipment ordered and will apply to fix the surface around the play area.

Description: Based on interviews and observation it was determined the program is not providing sufficient resilient surfacing under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
Evidence:
1. The swing set had no resilient surfacing under the seats or in the surrounding area.
2. The other fall zone areas also have low amount of resilient surfacing and does not meet the requirements.
3. There was not enough surfacing that could be raked around to create safe fall zones.

Plan of Correction: We will agree to keep children off the swings and we have ordered more surfacing.

Description: Based on record review and staff interviews, it was determined the center did not ensure ratios were met based on the youngest child in the grouping.
Evidence:
1. Staff interviewed about morning and afternoon transitions acknowledged that early mornings have the Toddlers and the Twos combined. The staff also said there have been late afternoons in which the toddlers were combined with the Twos.
2. Attendance and Head Counts sheets were reviewed and did find that on June 24, 2022 there was 1 staff with a group of 6 toddlers and two's for under 4 minutes. The Toddler ratio requires a 5 to 1 ratio.

Plan of Correction: We have made staff adjustments so ratios will be met.
