Comments:
A monitoring inspection was conducted on Thursday, August 4, 2022. The licensing inspector and licensing administer were onsite from approximately 11:00am until 12:00pm. In the center, there were 32 children present, ranging in ages from three months to six years with five staff members supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-80-A
Description: Based on observation, documentation review and interviews, the center did not ensure for each group of children, a written record of daily attendance documenting the arrival and departure as it occurs of each child was maintained.
Evidence:
1. The staff located in the Preschool classroom stated she had an attendance sheet for the classroom but as of 11:05am the attendance sheet had not been completed. Five children were observed present in the classroom. The attendance form did not document the arrival and departure of each child present.
2. The staff located in the Toddler classroom stated she did not have a daily attendance sheet.
3. Staff #1 stated she keeps an attendance list in one red notebook.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a safe and operable condition.

Evidence:
1. The gate on the chain link fence located on the playground was broken and not secure on all sides, creating head entrapment hazards.
2. The wooden fence contained four broken boards creating sharp edges that could entangle clothing or snag skin.

Description: Based on observation and interview, the center did not ensure that all fall zones were free of hazardous obstacles.
Evidence:
1. The center?s playground contained a swing set (approximately 6 feet 9 inches in height) sitting directly beside the playground fence.
2. Large toys were located approximately 10 feet directly in front of and behind the swing?s trajectory and on all sides of the slide/climbing wall/platform.
3. There was approximately 6 inches between Side A of the swing set and the fence.
4. There was approximately 7 feet 4 inches between Side B of the swing set and the fence.
5. There was approximately 11 feet 6 inches between the back of the swing set and the fence.
6. There was approximately 9 feet from the front of the swing set and wooden ground piece.
7. On August 4, 2022, staff stated the children had played on the playground that morning.

Standard #: 8VAC20-780-350-B-1
Description: Based on observation and interview, the center did not ensure the staff-to-child ratios were maintained whenever children were in care.
Evidence:
1. On August 4, 2022, the licensing inspector observed seven children (three months to 15 months of age) present in the Infant room being supervised by Staff #1 and Staff #2
2. Staff #1 was observed stepping out of the Infant classroom multiple times while the licensing inspector was on site to answer front door, make phone calls, prepare food and completing other various tasks for the center. Staff #2 was left alone in the classroom to supervise the seven infants.
3. The required staff-to-child ratio for children birth to 16 months of age is one staff person for every four children in care.
