Technical Assistance:
Technical Assistance provided: The new model form for Staff Orientation Training was provided to the Assistant Director. Also discussed swaddling and Lead Teacher Qualifications.

An unannounced complaint investigation was conducted on 8/3/22 in response to a complaint received in the licensing office regarding allegations of daily notes not being documented for Infants, supervision in the Infant classroom, parent not notified in writing of an injury to a child, new staff not being trained, and lack of Daily Health Observation training. Based on observation of the Infant classroom, a review of documentation, and interviews conducted, the allegations regarding new staff training and documentation of injury reports were supported and violations were cited. The allegations regarding Infant daily notes, supervision in the Infant room, and Daily Health Observation were unable to be substantiated due to insufficient corroborating evidence. Other violations that were not part of the complaint but were found during the inspection were also cited. If you have questions regarding this inspection, please contact the Licensing Inspector, Laura Brindle, at or 540-905-2062.

Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 8/16/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.
