Technical Assistance provided: The new model form for Staff Orientation Training was provided to the Assistant Director. Also discussed swaddling and Lead Teacher Qualifications.

Comments:
An unannounced complaint investigation was conducted on 8/3/22 in response to a complaint received in the licensing office regarding allegations of daily notes not being documented for Infants, supervision in the Infant classroom, parent not notified in writing of an injury to a child, new staff not being trained, and lack of Daily Health Observation training. Based on observation of the Infant classroom, a review of documentation, and interviews conducted, the allegations regarding new staff training and documentation of injury reports were supported and violations were cited. The allegations regarding Infant daily notes, supervision in the Infant room, and Daily Health Observation were unable to be substantiated due to insufficient corroborating evidence. Other violations that were not part of the complaint but were found during the inspection were also cited. Standard #: 8VAC20-780-160-A
Complaint related: No
Description: Based on review of one staff record, the center did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Evidence: The TB screening on record for Staff #1, with a start date of 3/21/22, was dated 4/20/22.

Plan of Correction: New hire employees will have TB screening within the 30 days prior to first contact with children.

Complaint related: Yes
Description: Based on review of one staff record, the center did not ensure that orientation training included: prevention of shaken baby syndrome and abusive head trauma, including procedures to cope with crying babies or distraught children; and prevention of and response to emergencies due to food and other allergic reactions. Evidence: The record for Staff #1 did not contain documentation of the training regarding the noted policies and procedures during orientation.

Plan of Correction: Orientation training topics will be updated to the new standard to include the policies notes.

Description: Based on review of one staff record, the center did not ensure that within 30 days of the first day of employment, staff received orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care. Evidence: The record for Staff #1, with a start date of 3/21/22, did not contain documentation of CPR orientation training.

Plan of Correction: Per the new standard; center will include CPR/First Aid training with staff orientation for those who are not certified upon hire.

Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place. Evidence: At approximately 10:40am three spray bottles were observed in the sink between the Infant and Toddler classrooms. One bottle was labeled ?soap and water?; another was labeled ?spray disinfectant?; and the last was not labeled but was similar in color to the spray disinfectant.

Plan of Correction: Classroom staff will ensure that each bottle of cleaner is clearly labeled, replacing labels as needed. The bottles notes were in a gated area without child access.

All cleaner bottles when not directly in a staff member's hand will be placed in a locked cabinet under the sink.

Description: Based on observation, the center did not ensure that recalled equipment was removed from the center. Evidence: Two original Bumbo seats were observed on the floor in the Infant classroom. Original Bumbo seats were recalled in 2012 and now require safety belts. The Bumbo seats observed did not contain safety belts.

Plan of Correction: We were not aware of equipment recall. Items were removed upon discovery.

Description: Based on observation, children?s hands were not washed with soap and running water or disposable wipes after eating a snack. Evidence: At approximately 10:23am an Infant was observed being removed from a high chair after eating a snack of crackers, cheese, and water, and being placed on the ground and being sent off to play without having their hands washed with soap and water or a disposable wipe.

Plan of Correction: Staff will review proper hand washing times and techniques.

Complaint related: Yes
Description: Based on review of documentation, the center did not maintain a written record of children?s serious and minor injuries in which entries are made the day of occurrence. Evidence: An incident report dated 3/17/22 was observed that contained documentation of a child hitting another child ?on the face leaving a mark?. There was no documentation of an injury report for the child who was hit on the face.

Plan of Correction: The parent of the child who was hit was present during the incident noted. The parent was also employed at the facility at the time of the noted incident, and stated she did not need written documentation. Staff will write injury reports for any injury circumstance, regardless of parent involvement with the injury.
