Technical Assistance:
Provided consultation on the following:
? Dates on bottles once bottles are made.
? Annual update requirements for emergency preparedness plan.
? Requirements for current, dated list with allergies, sensitivities and dietary restrictions for each room/area where children are present.
? Obtaining documentation of program lead qualification training or repeating the training after
hiring.

Comments:
An unannounced monitoring inspection was conducted today at 10:00am-12:00pm. There were 90 children ranging in age from 3 months to 12 years old supervised by 13 staff. The physical plant, outdoor playground area, programming, menus, 5 staff records, 5 children?s records, 2 medications, emergency drills, injury reports, and emergency supplies were inspected. Children were observed coloring, playing with table toys, playing video games, playing with blocks, napping and doing art. The center was compliant with staff trained in CPR/First aid, Daily Health Observation and Medication Administration at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-80-A
Description: Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.

Evidence: During inspection in the School Age/PreK combined classroom, there were 40 children present, but the written record reflected there were 38 children present. Staff were unaware of the total number of children in the classroom.

Plan of Correction: Two siblings arrived during the time the licensing inspection was being conducted in that room. Staff was in the process of supervising the children while also trying to answer licensing questions. Staff had not yet had the chance to update the attendance log with the children?s arrival. Staff will be re-trained on our child management procedures to ensure staff attendance is 100% accurate when a child arrives in a classroom. We will also coach staff that when VDOE licensing is on site that we need to delegate duties to other available teachers so that tasks are still completed while licensing inspector completes their inspection.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the laundry closet/kitchen off of the School Age/PreK combined classroom, the door was unlocked and cracked open and contained a jug of disinfectant that was accessible to children.

Plan of Correction: The door is secured with a keypad entry where a four digit code has to be input to unlock. The door also has an auto lock feature where the door is locked automatically after three seconds. Teacher stepped in the pantry area to put away morning snack. She pushed the door closed, but it failed to latch all the way.
As an additional security feature, we will install a self-closing device on the door so the door will close automatically once the door is egressed. Additionally, signage has already been installed that the door must be closed and locked at all times.

Standard #: 8VAC20-780-350-C
Description: Based on review of written attendance records, child file review, staff interviews, and observation, the center did not maintain the required mixed age group supervision ratio of staff to children where the ratio applies to the youngest child in the group.

Evidence: In the School Age/PreK combined classroom, there were 40 children present ages 4 years old-12 years old with only 3 staff members present. The staff to child ratio for 4 year olds is 1:10.

Plan of Correction: A teacher stepped out of the room for a brief moment to take a child to another classroom that just arrived at daycare. During that part of the day the Director usually walks children back to their classroom. However, the Director was not available as they were coordinating the VDOE licensing inspection with two on-site inspectors. The teacher came right back to the classroom. Staff has already been re-trained on the importance of maintaining ratios in the classroom. A formal teacher-to-student
ratio refresher training will be done with all staff at an upcoming staff meeting.
