Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:25 am-1:00 pm. There were a total of 115 children with 17 supervising staff members present. Children were observed during painting, puzzles, handwashing, bathroom breaks, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-780-70
Description: Based on record review, required records were not kept for each staff person.
Evidence:
1. Staff #2's record (Date of hire-05/02/2022) did not contain documentation of a second reference check and the fist reference check was completed after the date of hire.

Plan of Correction: Documentation of the references will be conducted prior to the first date of employment. Documentation will be added to the staff member's record.

Standard #: 8VAC20-780-240-E
Description: Based on record review and interview, staff did not complete orientation training in first aid and cardiopulmonary (CPR) as appropriate to the age of the children in care.
Evidence:
1. Staff #1, Staff #2, and Staff #3's records were missing documentation of training in first aid and cardiopulmonary (CPR).
2. Staff #4 stated that Staff #1, Staff #2, and Staff #3 did receive complete orientation in first aid and cpr, but did not complete documentation for the training.

Plan of Correction: The center will have the staff members to complete documentation of first aid and cpr orientation training.

Standard #: 8VAC20-780-240-I
Description: Based on record review, required records were not kept for each staff person.
Evidence:
1. Staff #2's record (Date of hire-5/2/22) did not contain documentation of a completed orientation.
2. Staff #3's record (Date of hire-7/6/22) did not contain documentation of a completed orientation.

Plan of Correction: The center will have the staff members to complete the documentation of the orientation.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas inside and outside, was not maintained in a clean, safe and operable condition.
Evidence:
1. In the Art Studio classroom, there was peeling paint located in the sensory table area.
2. In the Nature Center's classroom, there was peeling paint located near the exit door and an edge of a large area rug was rolled up on the edge. This rolled edge created a tripping hazard.
3. On the toddler playground, there was standing water in the sandbox area. A tarp that was used to cover the sandbox collected two inched of water. Also, there was a tire being used to hold the tarp down on the sandbox that collected six inched of water inside of the tire.

Plan of Correction: A work order will be put in to touch up the paint in the classrooms with peeling paint. The center will tape the rug ends down to avoid any tripping hazards. The owners will be contacted to fix the fence and a plastic cover will replace the tarp currently being used as a cover for the sandbox.

Standard #: 8VAC20-780-280-H
Description: Based on observation, a purse was accessible to children,
Evidence:
In the Trailblazers classroom(2 year old classroom) a purse was opened and located on a low shelf.

Plan of Correction: Purses will be placed into areas inaccessible to children.

Standard #: 8VAC20-780-550-D
Description: Based on review of documentation and interview, the center did not implement a monthly practice evacuation drill.
Evidence:
1. A monthly practice evacuation drill was not conducted for the month of July.
2. Staff #4 confirmed that a practice evacuation drill was not conducted for the month of July.

Plan of Correction: The center will conduct an extra evacuation drill in the month of August to make up for the missed evacuation drill.
