Technical Assistance:
Discussed the possibility of an intensive plan of correction, due to the risk of the violations.

Comments:
An unannounced virtual monitoring inspection was conducted, regarding a center report of 3 children leaving the premises. After reviewing all the available information violations were found. A Violation Notice was given to the center. The ongoing compliance with the standards is the responsibility of the licensee. Please contact me if you have any questions regarding this inspection.

Violations:
Standard #: 8VAC20-780-340-A
Description: The center failed to ensure care guidance and protection when Child 1, Child 2 and Child 3, left the center on 7/22/22.
Evidence: Based on the review of records and interviews, the center staff was unaware that Child 1, Child 2 and Child 3, had left the center and failed to have a system in place to ensure that children could not leave the facility. The center failed to have staff at the front of the building which made it possible for Child 1, Child 2 and Child 3, ages 9, 10 and 13 to leave the center and walk down to the nearby Starbucks. Teacher 1 noticed they were missing after 7 minutes and went to the hallway door to notify the person in charge, Teacher 2. At that time Child 1, Child 2 and Child 3, walked back up the hallway after being discovered in the hallway by Teacher 2 as they returned to the center.

Plan of Correction: We will install a door alarm in the kitchen exit door. The kitchen is not in use all throughout the day, so with the alarm installation, we will be alarmed when somebody exits/steps out.

Standard #: 8VAC20-780-340-D
Description: The center failed to have a qualified lead in the school age grouping.
Evidence: Based on interviews and review of records, the school age room did not have a lead teacher present after three o?clock on 7/22/22. The room does not have a qualified lead if the ratio is met with one teacher. The lead, Teacher 2, is only present in the room if the room has more than 18 children present. Teacher 1 is the main teacher in the room.
Teacher 1 does not meet the qualifications of a lead.

Plan of Correction: Reviewed lead teacher qualifications and ensured that a lead qualified teacher was always present in the room.

Description: The center failed to keep a child under 10 in sight and sound supervision, a 9 year old , Child 1, left the center on 7/22/22 and went to Starbucks, 179 yards away.
Evidence: Based on interviews with staff and review of records, on 7/22/22 at approximately 3pm Child 1, Child 2 and Child 3, asked the school age teacher, Teacher 1, if they could build a basketball structure. After starting to build the structure in the room they realized that they did not have enough space inside. They went outside to build it and Teacher 1 left the door open to the playground so he could see and hear them. There was an altercation between Child 4 and Child 5, in the room at 3:15pm Teacher 1, went to talk to Child 4 and Child 5. During that time Child1, Child 2 and Child 3 came in and said they were going to get water. The school age group had moved to a different classroom due to AC issues and Teacher 1 let them go to their classroom to retrieve water bottles. Child 1, Child 2 and Child 3 went down the hall and left the building by going through the kitchen. They walked 179 yards to the Starbucks down the street and returned approximately 7 minutes later. Teacher 2, observed Child 1, Child 2 and Child 3 walking up the hallway to the classroom with Starbucks cups. Teacher 2 asked where they had been and they told her they had gone to Starbucks to get ice water. At the time of their return Teacher 1 was looking out the door to let someone know he was missing Child 1, Child 2 and Child 3.

Plan of Correction: We will be keeping all kids grouped together and in sight and sound, all in the same location inside or outside not in two different places, we will increase the number of name-to-face checks done throughout the day not just during transitions.

Standard #: 8VAC20-780-340-G
