Comments:
An unannounced monitoring inspection was conducted on 8/18/2022. The school was closed for care in order to complete in-service training for staff. The Inspector reviewed compliance in the areas of administrative records, emergency preparedness, caregiver training, children's records, staff records, and the physical plant.

Description: Based on a review of children's records, it was determined that two records do not contain all required information.
Evidence:
1 - Child record A does not include documentation of an annual parent review of the contact information. This file was most recently reviewed on 7/19/2021.
2 - Child record B does not include: the addresses for each emergency contact person; documentation of an annual parent review of the contact information. This file was most recently reviewed on 7/23/2021.

Plan of Correction: Both parents will be asked for the missing information.
