An unannounced monitoring inspection was conducted on 7/28/2022 10:00 a.m. - 4:45 p.m. There were 77 children present on 07/28/2022 ranging in ages from 2.5 months to school age with 12 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of 8 child records, 3 medication records, and 6 staff records were reviewed. The information gathered during the inspection determined violations with applicable standards. A violation notice was issued.

Upon entering the Infant 1 classroom, S1 was observed to be in the classroom by herself supervising 4 infants ranging from 10 to 15 months. During observations of the Toddler A and B classrooms on 07/28/2022, licensing inspector observed a clipboard with 5 injury reports dated 07/21/2022, 07/22/2022, 7/26/2022, and 07/27/2022.
