Comments:
A monitoring inspection was conducted on 08/17/2022. Upon arrival there were 16 children in care and 4 staff present. The inspector conducted a tour of the facility, made observations of the program and conducted a record review. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 1 staff record, along with any requested program records submitted by the facility to determine if required documentation was complete. The information gathered during the inspection determined no violations with applicable standards or law. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on a record review the center did not ensure all staff had the required background checks.
Evidence:
Staff #1 (hired 05/19/2022) did not have documentation of the results of a central registry check.

Plan of Correction: The following corrective action was provided by the Owner/Director during the exit interview: The check was sent, however it came back needing more information. A follow-up call will be made to check on the status of the background check.

Standard #: 8VAC20-780-60-A
Description: Based on a record review the center did not ensure each child had all required information in their record.
Evidence:
1. Child #1 (enrolled 05/02/2022), did not have complete information for 2 required emergency contacts.
2. Child #2 (enrolled 07/12/2022) did not have complete information for 1 of the 2 required emergency contacts.

Plan of Correction: The following corrective action was provided by the Owner/Director during the exit interview: The Director/Owner will obtain this information from the child's parent and document it in the record.

Standard #: 8VAC20-780-260-B
Description: Based on a record review the center did not ensure there was an annual health inspection competed.
Evidence:
The last health inspection for the facility was dated 07/08/2021.

Plan of Correction: The following corrective action was provided by the Owner/Director during the exit interview: the health department will be contacted this week to schedule an inspection. The call will be documented.

Standard #: 8VAC20-780-330-B
Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.

Evidence:
On the playground, two children were observed on a portable see-saw. There was no resilient surfacing underneath this piece of equipment.

Plan of Correction: The following corrective actions were provided by the Owner/Director during the exit interview.
The see-saw will be removed from the playground until resilient surfacing can be placed underneath it.

Standard #: 8VAC20-780-430-K
Description: Based on observation the center did not ensure there was individual space for each child's belongings.
Evidence: Some of the children's book bags were hung on hooks with no identifying space for each. Other bags were observed, group together on the floor.

Plan of Correction: The following corrective action was provided by the Owner/Director during the exit interview: Provision will be made for an individual space for each child's belongings.

Standard #: 8VAC20-780-440-E
Description: Based on observation the center did not ensure there was at least 12 inches of space between cots when children are resting.
Evidence:
Children were observed napping on cots in the front room. There was not at least 12 inches of space between the cots, the children were napping on.

Plan of Correction: The following corrective action was provided by the Owner/Director during the exit interview: Tape will be used to mark where cots should be placed to ensure there is at least 12 inches of space between each during nap.
