An unannounced complaint investigation was conducted on 08/17/2022 and 08/24/2022 regarding special care provision and emergency and staffing and supervision. Interviews were conducted and records were reviewed. In regards to complaint it was deemed not valid. There was non complaince found during the inspection.

Standard #: 8VAC20-780-550-P Complaint related: No Description: Based on record review,observations and staff interviews, the licensee did not ensure the written record of children's serious and minor injuries in which entries are made of the day of occurence contained all required documentation.
Evidence: On August 17, 2022, the written record for injury provided did not have a type of circumstance of the injury . The report from August 16, 2022 stated " During clean up Child #1 went to sit on carpet " the report did not provide details of the incident. Staff #1 confirmed the report did not provide the details of the incident. Staff #1 did complete the documentation when the licensing inspector was present.Plan of Correction: Management and all staff will review all expectations for appropriate incident descriptions on or before 9/6/22. DM will review the incident/accident log at all school visits to ensure compliance.
