Comments:
The licensing inspector conducted an unannounced complaint inspection on 08/25/2022 in response to a complaint that was received in the licensing office on 08/23/2022 regarding allegations in the area of emergencies. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on observations, interviews and record review, the center did not ensure children under 10 years of age were always within sight and sound supervision of staff.

On 08/225/2022 around or about 1:30pm, thirteen 3 year-old children were left unsupervised during nap. The inspector observed Staff #2, who was responsible for the group of children, outside of the classroom attending to administrative duties. When asked why she left the classroom unsupervised, Staff #2 replied she thought she could leave the children unsupervised while napping and stated, "how else are we supposed to get things done?"

Plan of Correction: All staff will be retrained on safety protocols in the employee training manual that
outlines Supervision of Children, Identifying Where Children are at all Times and Injured Child procedures by close of business September 9, 2022. In addition, management will be including the listed below training videos which
addresses monitoring, supervision and behavioral guidance for preschool settings for
existing staff and new hires going forward. Staff will be required to complete this training on an annual basis going forward and/or as needed.

3. The Diverse Role of the Teacher?s Assistant (video training)

Description: Based on interviews and a record review the center did not ensure the 2 year-old ratio of 1 staff member for every 8 children, was maintained.

Evidence:
1. On 08/22/2022 around or about 11:00am the 2 year-old room was out-of-ratio when staff #2 left the classroom to attend to something in another classroom. Both staff #1 and #2 confirmed there were 14 children in care with 1 staff at the time staff #2 left.

2. On 08/25/2022 the inspector observed staff #1 leave the 2 year-old classroom to attend to a task in the bathroom area, leaving the classroom out of ratio with 14 children and 1 staff member.

3. During interviews with staff it was confirmed that leaving the classrooms out of ratio to perform tasks is a common practice within the center. One staff member stated, "How else are we supposed to get things done?"

Description: Based on interviews with staff and record review, the center did not ensure the parent was immediately notified their child sustained a serious injury.

Evidence:
On 08/22/2022 a child tripped and sustained a cut on the face that required outside medical care. The incident reports indicates the child sustained the injury at 11:00am and that the parent was not notified immediately, but called by telephone at 12:08pm.

Plan of Correction: All staff will be retrained on safety protocols in the employee training manual that
outlines Supervision of Children, Identifying Where Children are at all Times and Injured Child procedures by close of business September 9, 2022. In addition, management will be including the listed below training videos which
addresses monitoring, supervision and behavioral guidance for preschool settings for
existing staff and new hires going forward. Staff will be required to complete this training on an annual basis going forward and/or as needed.
