Comments:
An unannounced renewal inspection was conducted today with the site manager, who was present with a second staff and 8 children, ages ranging from 5 to 11 years. The inspection took place from 4:10 pm to 5:05 pm. The children were engaged in free play at the beginning of the inspection, for their snack, they were provided cinnamon graham crackers and milk.
Three children had medications on site and two allergy care plans were reviewed.
Seven children and three staff files were reviewed as well.

Violations:
Standard #: 8VAC20-780-90--A
Description: Based on a review of 7 children's files and an interview with Administrative staff, it was determined that a written agreement between the parent and center to address authorization to seek emergency care, pick up if a child becomes ill and notification if someone in the child's household contracts a reportable communicable disease.
Evidence:
There was no written agreement on file for child A through G. Child C, D, E, F and G all enrolled on 8/10/22. Child A enrolled on 8/15/22 and child B enrolled on 8/16/22.

Plan of Correction: I will make sure parent sign off on the agreement and place the agreement in each child's file.

Standard #: 8VAC20-780-420-A
Description: Based on a review of 7 children's files and an interview with Administrative staff, it was determined that written policies and procedures outlined in this standard were not given to parents upon enrollment.
Evidence:
Parents were not provided written policies and procedures for child A through G.

Plan of Correction: I will have parents sign receipt of the policies and procedures and place in each child's file.
