Comments:
An unannounced complaint investigation was conducted on 8/12/2022 and 08/17/2022 regarding programs. Video footage was reviewed, interview was conducted, observation were conducted and records were reviewed. The preponderance of evidence support the allegations in regards to programs and the complaint has been deemed VALID.

Description: Based on video footage , staff interview and documentaiton, the licensee did not ensure behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Evidence:

Video footage was reviewed with Staff #2.

The video footage observed shows, in the toddler room, Staff #1 is sitting between occupied cots. Child #1 is on a cot while laying on her back moving her legs and arms around. Staff #1 adjusted child#1 on the cot by pulling the child's right forearm and putting it across her body and moving her shoulder . This movement caused child #1 to turn over on her stomach with her knees tucked. Staff #1 adjusted the child's legs and feet and tossed the blanket completely over the child covering her whole body.

Staff #1 moved to sit next to another child's cot and pulled child #1 entire cot, with child #1 still on it, to move her closer.

Approximately two minutes later Child #1 is on her back while moving around on her cot and legs and arms moving . Staff #1 pulls child #1 up by her bicep to turn child #1 on her side and then tosses the blanket on top of child#1 which covers the child from head to toe.

Staff #3 provided written statement that indicated that Staff #1 was observed on video improperly moving a child on their cot during nap time.

Plan of Correction: Staff has been retrained as of 8/8/22 on proper movement of children and naptime supervision guidelines. School management and DM will do unannounced classroom observations both in person and via camera reviews to ensure compliance. This topic will also be covered at our September staff meeting for all school staff.
