An unannounced monitoring inspection was conducted at 10:30am. Observed 27 children + 5 direct-care staff + 3 additional staff preparing lunch and covering classes as needed. Ratios were in compliance at the time of inspection. Children were observed participating in a magic show, while infants were being held and napping. The facility was found to be clean and well supplied with toys and equipment for the children. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Standard #: 22.1-289.035-B-4 Description: Based on records reviewed, it was determined that 1 of 5 staff records did not include a required background check that was to have been completed prior to the 1st day of employment. Evidence: Staff #1 ? There was no documentation of a completed out-of-state sex offender registry check. Date of hire 6/6/22. Plan of Correction: The inspector showed the Director how to obtain the sex offender registry check, and it was completed during the inspection.
Standard #: 8VAC20-780-210-A Description: Based on records reviewed and observations made, it was determined 2 of 5 staff records did not include documentation needed to qualify them for the Lead Teacher positions they were working. Evidence: Staff #9 ? There was no documentation of the staff person having a high school diploma. Date of hire was 4/9/18. Plan of Correction: Not available online. Evidence: Staff #5 ? There was no documentation of the staff person having a minimum of 6 months supervised program experience. Date of hire was 5/16/22. Plan of Correction: Not available online. Evidence: Outlets in 1 of 5 classrooms, in the hallways, and lobby area did not have protective covers. Plan of Correction: The outlet in the classroom was covered during the inspection.
Standard #: 8VAC20-780-340-D Description: Based on observations made and records reviewed, it was determined that 2 of 5 classrooms/groups of children were not supervised by a Lead qualified staff person.

1. The infant room/group and school-age room/group were not supervised today by a staff person with documented qualifications to work as a Lead Teacher.
