Comments:
The Licensing Inspector initiated a monitoring inspection on 08/24/2022 and completed the inspection on 08/25/2022. The inspector was on site from 10:00am until 11:45am on 8/24/2022, and from 2:55 to 4:45 on 8/25/2022.

There were 23 children under the care of 2 staff. The ages of the children ranged from five to ten years old. The children were observed during outside time and indoor play. Five children?s record, and three staff records were reviewed. The inspector also took measurements on classrooms that had been added. These measurements will support the capacity modification the provider has requested.


Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to have area outside maintained in a clean, safe and operable condition.

Evidence:

1. On the playgrounds blacktop, there were 4 screws protruding out of the ground in a square shape. Children were observed playing in this area. This is an impalement risk to children.

2. One the playground blacktop, there were to basketball goals laying on the ground not in use. The goals had screws protruding out of it about an inch. This is an entanglement hazard for children.

3. On the back area of the playground there was a broken metal gate laying on the ground. This is and entanglement hazard for children.

4. In the mulched playground area where children were observed playing, there was a metal rod on the ground. The rod was about 4 feet long. This is an impalement hazard for children.

Plan of Correction: #1 Maintenance has been requested and will be putting up goals on 9-6-2022.
#2 Maintenance has been requested and will be taken care of on 9-6-2022.
#3 Maintenance has been requested and will be taken down on 9-6-2022.
#4 The rod has been removed from the ground 8-25-2022.
