Comments:
An unannounced monitoring/renewal inspection was conducted on 08/22/2022 from 12:40pm to 4:25pm. There were 147 children present ages 5 months to age 5 years old supervised by 19 staff. The physical plant, outdoor playground area, programming, menus, 5 staff records, 5 child records, 8 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in art, reading, napping and transitioning from nap. Diapering and handwashing procedures were also observed. There were adequate staff with current certification in MAT, CPR and First Aid, and DHO training. Staff to child ratios were in compliance during the inspection. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of 5 staff records, the center did not ensure at the time of employment that each staff member had documentation of a negative tuberculosis screening.

Evidence: The record for Staff #5 (Date of Hire 8/2/21) did not contain documentation of a negative tuberculosis screening.

Plan of Correction: All new employees will have a TB test on file before 1st day.

Standard #: 8VAC20-780-80-A
Description: Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.

Evidence: During inspection in the Nursery 2 classroom, there were 15 children present, but the written record reflected there were 17 children present.

Plan of Correction: Teachers will be reminded to sign children out when they are no longer in care.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence: The slide on the Toddler playground has a large crack on the side where a child could cut themselves or snag skin.

Plan of Correction: The slide will be repaired and playground maintained.

Standard #: 8VAC20-780-330-B
Description: Based on observation, the center did not ensure playground equipment has enough resilient surfacing to comply with minimum safety standards as described in the American Society for Testing and Materials standard F1292-99.

Evidence:
1)On the School age playground, the play structure is 8 feet off the ground with mulch resilient surfacing of only 1-2 inches in fall zone areas surrounding the structure. The required depth of the mulch is at least 9 inches.

2)On the School age playground, the Monkey bars are 6 feet and 6 inches off the ground with mulch resilient surfacing of only 2-3 inches in the fall zone area. The required depth of the mulch is at least 9 inches.

3)On the Threes and Fours playground, the tall slides are 5 feet and 6 inches off the ground, the curved slide is 3 feet and 8 inches off the ground, and climbing area is 5 feet off the ground with mulch resilient surfacing of only 1-2 inches in each of the fall zone areas. The required depth of the mulch is at least 6 inches.

4)On the Twos playground, the tall slide is 4 feet and 6 inches off the ground and the smaller slide is 2 feet and 6 inches off the ground with mulch resilient surfacing of only 1-2 inches in each of the fall zone areas. The required depth of the mulch is at least 6 inches.

5)On the Toddler playground, the 3 slides are 2 feet and 6 inches off the ground, with mulch resilient surfacing of only 2-3 inches in each of the fall zone areas. The required depth of the mulch is at least 6 inches.

Plan of Correction: School will follow up with existing facilities request for expedited supply of mulch to meet and maintain the required depth of mulch in all fall zones.

Standard #: 8VAC20-780-520-C
Description: Based on observation, the center did not ensure that diaper cream was inaccessible to children.

Evidence: In the Early Preschool 2A and in the Early Preschool 2B classrooms, diaper cream was stored in the bottom cabinet of the diapering station where the doors were unlocked and the diaper cream was accessible to children.

Plan of Correction: All diaper cream will be kept inaccessible to children. Cabinets will be locked.
