Technical Assistance:
The following topics were discussed:
* The director will ensure to document the dates when changes and updates are made to the Emergency Preparedness Plan.
* The director will ensure to follow up on VA and out-of-state background checks that are pending, and submit the results to the licensing inspector.
* Buggies used to transport infants must be maintained clean.

Comments:
A monitoring inspection was conducted on 09/06/2022 between the hours of approximately 11:45 am and 2:45 pm. There were 40 children present, ranging in ages from 6 month to 4 years, with 12 staff supervising. Children were observed while cleaning up after lunch and transitioning to nap time.
Staff was observed while changing diapers, cleaning up and reading stories.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-40-M
Description: Based on observation, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8 was not available in each room or area where children are present.
Evidence:
1) Although allergy a list with children's allergies and dietary restrictions was available in each classroom, these lists included only the children in that specific classroom and not all children as required.

Plan of Correction: Allergy list will be updated to have every child in the school posted in each room.

Standard #: 8VAC20-780-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The records for Child #1, Child #2 and Child #3 did not contain first date of attendance.

Plan of Correction: We will update all child files to have the first date of attendance.

Standard #: 8VAC20-780-70
Description: Based on record review, when a reference to character and reputation as well as competency was taken over the phone, not all required information was documented.
Evidence:
1) The records for Staff #1 and Staff #2 did not contain the date of contact that the reference check was taken.
2) The record for Staff #3 did not contain the date of contact that the reference check was taken and the signature of the person making call.

Plan of Correction: Moving forward all reference checks will have date they gave the reference.
