Comments:
A renewal inspection was conducted Tuesday, September 6, 2022. The inspection was initiated at approximately 10:30 AM and concluded at approximately 1:00 PM. There were 40 children present, ranging in ages from three months to five years, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on review of five children?s records and interview, the center did not have the required information to two children?s records.

Evidence:
1. The record for Child #1 (start date 08/29/2022) was missing the physical address for one of the two emergency contacts listed in the record.
2. The record for Child #4 (start date 09/06/2022) was missing the physical address for one of the two emergency contacts listed in the record.

Plan of Correction: I will request the addresses for the emergency contact and add the information to the records.

Standard #: 8VAC20-780-60-A-8
Description: Based on review of five children?s record and interview, the center did not have a written allergy action care plan for one child diagnosed with a food allergy, to include instructions from a physician regarding the steps to take in the event of an allergic reaction.

Evidence:
The record for Child #1 (start date 08/29/2022) indicated the child has a food allergy. Staff #5 stated the parent has not submitted the allergy action plan.

Plan of Correction: We have requested this information from the parent. I will follow up with the family today to obtain the information.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not maintain all areas and equipment of the center, inside and outside, in a clean, safe and operable condition.

Evidence:
Four tricycles located on the side outdoor playground, were missing the pedals, leaving the metal spokes exposed.

Plan of Correction: The tricycles were removed from the playground while the inspector was onsite.
