Comments:
Today an unannounced Monitoring Inspection took place between the hours of 12:35pm and 2:00pm. There were 3 classrooms observed with a total of 20 children with 4 staff. The children were observed taking a nap, drinking bottles and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on a review of staff records, 2 out of 2 staff did not have a central registry finding within 30 days of employment.

Evidence:
1. Staff #1 (doh: 7/12/2022) had a central registry finding on file that was dated 8/21/2022.
2. Staff #2 (doh: 4/1/2022) had a central registry finding on file that was dated 5/6/2022.

Plan of Correction: Moving forward, we will get the central registry within 30 days or have documentation of follow-up.

Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, 1 out of 2 staff members did not submit a negative tuberculosis screening by day 1 of employment or 30 days prior to employment.

Evidence:
1. Staff #2 (doh: 4/1/2022) had a TB screening dated 5/15/2022 on file during the inspection.

Plan of Correction: We will make sure all new staff get their TB screening by day 1 of employment or within 30 days prior.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.

Evidence:
1. The metal gazebo on the playground was observed to be rusted, missing nails, broken in one area, and wobbly.
2. The wooden play structure on the playground was observed to have protruding, loose nails and broken/splintering pieces of wood.

Plan of Correction: We will remove the gazebo and fix anything that is broken.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation, the 3 year old class was not within the allotted staff to child ratio.

Evidence:
1. Upon arrival to he facility at approximately 12:35pm, it was observed that a staff member was sitting in between the 4's classroom and the 3's classroom. The staff member was facing the children in the 4's classroom and the door was open. The 3's classroom door was closed. The staff member stated that they were watching both classrooms due to the 3's teacher going on their lunch break at 12:00pm. When the licensing inspector entered the 3's classroom, there were 11 children with no staff members to supervise them as they napped. At approximately 12:40pm, the Director came into the 3's classroom to supervise the children until a staff member returned from their lunch break. The classroom was out of ratio for approximately 40 minutes.

Plan of Correction: This was corrected during the inspection. Our ratio issue will be resolved tomorrow. We had a staff member out today 8/31/2022.

Standard #: 8VAC20-780-570-B
Description: Based on observation, bottles were used while a child was in his designated sleeping location.

Evidence:
1. Child #2 was observed to be drinking a bottle in their crib when the inspector entered the infant classroom.

Plan of Correction: The bottle was removed during the inspection. We will not give bottles in the cribs moving forward.
