Technical Assistance:
1. Discussed safety requirements of covering outlets in areas children have access to, and keeping cleaning supplies out of reach of children. As children get older and can reach higher places hazardous items will need to be moved higher.
2. Forms provided: emergency contact, emergency preparedness plan in Spanish, and attendance.

Comments:
10:00-11:45am A scheduled initial subsidy inspection conducted. Observed 3 children in care = 6 points with the vendor. Ratios and supervision were in compliance. Children were coloring and playing with toys and blocks during the inspection. The vendor was talking and playing with the children on the floor. One child was reading the alphabet in English. Conversations were in Spanish between vendor and children. The inspector used translation app to communicate with the vendor. The childcare areas of the home were found to be very clean and sufficiently supplied with toys and equipment for the children. The vendor is currently certified in First Aid + CPR. Non-compliances found.

Violations:
Standard #: 22VAC40-665-170-B
Description: Based on records reviewed, it was determined that 3 of 3 children's records did not include required information.

Evidence:

1. Child #1 - The child's home address, proof of identity (birth certificate); parents name, address and phone numbers for home and work; and name, address and phone number for an emergency contact person (other than parents).

2. Child #2 - Proof of identity (birth certificate); parents work address and phone numbers; address for the emergency contact person listed; and emergency medical authorization.

3. Child #3 - The child's home address, the father's home/work address and phone number, and mother's work address and phone number; address for the emergency contact person listed, and emergency medical authorization.

Plan of Correction: The vendor prepared emergency medical forms to be signed by parents, and said should would request birth certificates for children today. The other needed information will be filled out by parents and kept on file. Inspector will provide forms.

Standard #: 22VAC40-665-390-A
Description: Based on observations made and interview conducted, it was determined that there was no thermometer for taking the children's temperature when needed in the home.

Plan of Correction: The vendor will buy a thermometer.
