Technical Assistance:
Discussed 10 written permission for medication when waiting for physician authorization to avoid outdated authorization.

Comments:
An unannounced monitoring inspection was conducted today from2:30-3:35. There were 50 children directly supervised by staff. The physical plant, 2staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating snack. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-70
Description: Based on record review of staff #1's file, it was determined the file did not contain a signed Sworn Disclosure Statement and complete orientation documentation.
Evidence:
1. Staff 1's Sworn Disclosure Statement was not signed and dated.
2. Staff 1's Orientation documentation did not contain the staff's signature acknowledging individual had complete it.

Plan of Correction: We will review all staff files to ensure signatures are there and be sure to review paperwork from each staff.

Standard #: 8VAC20-780-270-A
Description: based on observation, it was determined the support beams outside on the playground were not maintained in a safe manner.
Evidence:
1. The beams around the playgrounds perimeter had bolts in at least 5 places that were pushing up and out of the beams which posed a protrusion hazard.

Plan of Correction: We will have maintenance come and fix all of the protruding bolts

Standard #: 8VAC20-780-340-F
Description: Based on observation, it was determined a 4 years old child was not within sight and sound supervision of a staff member when the child was found walking from the water fountain in the center's hallway.
Evidence:
1. The fours group was seated in the corner back room and the children were eating snack.
2. As the Director and Inspector were walking towards the front of the building, a 4 years old child was carrying a plastic cup of water while walking towards the back of the center.
3. The four's class had one teacher in that classroom and that staff member did not have sight of the child.

Plan of Correction: Staff will all be reminded that children may not walk to and from water fountain or bathroom without direct supervision of staff.

Standard #: 8VAC20-780-550-P
Description: Based on record review of recent accident reports, it was determined that one accident report did not include a plan to prevent recurrence.
Evidence:
1. An accident report from July 12, 2022 listed an accident in which parent had to be contacted but the plan of correction was not complete.

Plan of Correction: We will continue to review accident reports and remind staff the importance of completing the reports.
