10:40-11:50am Annual unannounced subsidy inspection completed. Observed 2 children = 4 points with the vendor. Ratios and supervision were in compliance. Children were playing with toys, watching child appropriate cartoons on tv, and playing with the vendor during the inspection. The childcare areas were found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. The main childcare area has been moved to the mid-level of the home. The vendor is current certified in First Aid/CPR. Recordkeeping non-compliances found.

Evidence:
1. Child #2 - Name, address and phone number of an emergency contact person. A parent was listed.
2. Child #7 - Written permission for emergency medical authorization.Plan of Correction: The vendor said she will have parents provide the required information.
