Technical Assistance:
Review of site files was conducted. Discussed with center director:
-Staff qualifications
-Annual training hours

Comments:
An announced monitoring inspection was conducted today from 10:00am-11:40am. Staff files were reviewed at the Loudoun - YMCA's main office in Leesburg. Two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review, the program did not obtain within 45 days of employment a search of the child abuse and neglect registry or equivalent registry from a state in which the individual has resided in the preceding 5 years.

Evidence:
Staff #1's (DOE: 8/17/2016) record contains an out of state central registry check that was obtained on 8/8/2022.

Plan of Correction: Background checks were submitted on 9/23/2022. We have also reached out to HR to inquire about employee files who have been with our program longer than 5 years to maintain record of initial screenings including child abuse and neglect central registry searches.

Standard #: 8VAC20-770-60-C-2
Description: Based on record review, the program did not obtain a central registry check for a staff member within 30 days of employment.

Evidence:
Staff #1's record does not contain a central registry finding. The staff member's date of hire was documented as 8/17/2016.

Plan of Correction: Background checks were submitted on 9/23/2022. We have also reached out to HR to inquire about employee files who have been with our program longer than 5 years to maintain record of initial screenings including child abuse and neglect central registry searches.

Standard #: 8VAC20-780-245-M
Description: Based on record review, the programdid not keep documentation of training in a manner that allows for identification and that includes, training delivery method and the number of training hours.

Evidence:
Staff #1's record contains 8 training certificates that do not indicate the number of training hours or the training delivery method.
Staff #2's record contains 6 training certificates that do not indicate the number of training hours or the training delivery method.

Plan of Correction: We have updated our documentation method to track training received by employee throughout the duration of their career. The document will contain the name or type of training along with hours received and delivery method. We have also reached out to our corporate office to gather all documents pertaining to employees who have been with the facility over the past few years and will maintain those records at the localized branch.
