An unannounced investigation was conducted on 9/1/2022, in response to a self-reported incident received by the Fairfax licensing office regarding areas of the standards related to programs. Based on interviews, record review, and observations made, the preponderance of evidence does not support a violation, however, non-related violations were documented on the violation notice issued to the facility.
