Technical Assistance:
The next anticipated inspection will be the providers renewal inspection. Provider was reminded to have all renewal application documents completed and sent into licensing office 45 days prior to expiration for processing. The renewal application is available on the VDOE website.

Comments:
Today an unannounced Monitoring Inspection took place between the hours of 9:50am and 1pm. There were 7 classrooms observed with a total of 77 children with 19 staff within the supervision guidelines. The children were observed playing on the playground, transitioning into the classroom from outside, learning about Johnny Appleseed and other organized activities. Positive interactions between the staff and children were observed. A complete inspection of the physical plant, children and staff records, medications and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on record review and interview, the center did not obtain a sworn disclosure statement for each employee prior to the first date of employment.
Evidence: Staff #4 (date of hire: 8/23/2022) record did not contain documentation of a sworn disclosure statement. The staff member confirmed that they had not submitted a sworn disclosure statement.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-780-160-A
Description: Based on record review requirements for tuberculosis (TB) screenings were not met.
Evidence:
1. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.
2. Staff #3 (date of hire: 5/4/2022) TB screening is dated 5/20/2022 and Staff #4 (date of hire: 8/23/2022) TB screening is dated 9/10/2022
3. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
4. Staff #2 (date of hire: 8/25/2022) TB screening is dated 11/24/2021 and Staff #5 (date of hire: 8/15/2022) TB screening is dated 9/16/2021.

Plan of Correction: We will adjust our school policies so that staff know TB screenings must be provided prior to working with the children and that they can't be more than 30 days old.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation, unused electrical outlets did not have protective covers.
Evidence: There was an outlet cover missing from an outlet in the hallway across from the infant cubbies.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-780-510-G
Description: Based on observation, medication was not labeled with each child's name.
Evidence: Child #6 and Child #7's medication was not labeled with their names.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-780-510-P
Description: Based on observation, when medication authorizations had expired the medication was not returned to the parent or disposed of.
Evidence: The medication authorization for Child #6s medication was signed by the parent on 6/2/2022 and expired 6/12/2022. The medication was observed on site during todays inspection.

Plan of Correction: I will contact the parents today by email and I'll provide them with the form to be completed by the doctor.
