-Standard 350B, 350C and 350 I were discussed with the director. Please note that 350I does not apply to children under the age of 16 months.
- Comments:

A monitoring inspection was conducted on 09/07/2022. There were 13 children present and 3 staff. A tour of the facility was conducted, interviews with staff and records were reviewed. The inspector emailed the director. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Based on a record review the center failed to maintain all the required information for each enrolled child.
Evidence:
1. Child #1, enrolled on 09/06/2022 did not have documentation of a staff member who viewed the child's proof of identification or 2 complete emergency contacts that include name, full address and phone number.
2. Child #2, enrolled 08/01/2022 did not have documentation of who viewed the child's proof of identification or 1 of the 2 required emergency contacts that include name, full address and phone number.Plan of Correction: The director will obtain this information and document it in the record.
Standard #: 8VAC20-780-280-B Description: Based on observation the center did not ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the main daycare room towards the bathrooms there were 2 bottles of sanitizer and 2 bottles of bleach wipes that were not locked on a shelf above the sinks.
2. In the infant room there was a bottle of disinfectant near the changing table.Plan of Correction: The following corrective action was provided by the director during the exit interview:
The director will remind staff that all chemicals and hazardous substances must be kept locked when not in use.
Standard #: 8VAC20-780-350-C Description: Based on observation the center did not ensure when children are in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest in the group shall apply to the entire group.
Evidence:
1. When the inspector arrived there were 13 children napping in the main daycare room.
2. The youngest child in the room was 6 months-old, making the required ratio 1 staff member for every 4 children.
3. There was only 1 staff member supervising the children. The second staff member #1 was observed on the floor sleeping with a blanket over her body.
4. The director confirmed that was not the first time staff #1 was found napping on the floor instead of supervising the children.Plan of Correction: The following corrective action was provided by the director during the exit interview.
The center has been very short staffed. The director will look at nap time and separate the children to maintain ratio. Staff will be retrained on supervision and ratios.
