Technical Assistance:
Discussion that the other medication observed on site for Child #1 was set to expire at the end of September 2022. Staff stated that they were aware of this and has already notified the parent.

A review of the Children's Program Quick Access links form was conducted. It appears that this new guide is primarily for the on-site CASA staff to utilize.

Discussion that 7 of the children enrolled in the Forest Grove CASA and Camps program are transported to the site from Sully Elementary School.

Discussion of Child #6 and the completion of the Health and Skill form completed by the child's parent. Based on the information recorded on this form under the category of Disability and Medical Needs (general safety), that parent rated her child as "4" (the scale was 1 to 4 in terms of need) for full supervision, with one on one supervision. Based on observations made today, Child #6 was not receiving one on one supervision as indicated by the child's parent for the child's disability and medical needs (general safety). Currently their were 35 children enrolled (including Child #6) with a total of 2 staff supervising the entire group.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 09/14/2022, between the hours of 2 pm to 3:30 pm. There were 32 school-age children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 6 children records were reviewed. Staff records were reviewed June 23, 2022.

Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the center.

Standard #: 8VAC20-780-510-E
Description: Based on review, 1 of 2 medications indicated that the procedures for maintaining medications on site were not in compliance.

Evidence -
1. On the date of inspection (09/14/2022), the duration of the medication authorization (10 day short form) on file at the center for Child #2 had expired.
2. The duration of the form was listed as: 08/25/2022 through 09/03/2022.
3. The medication was observed still on site within the locked medication storage unit.

Plan of Correction: Will return to parent of Child #2.
