Technical Assistance:
Information provided regarding the background check portability request system and the provider portal account.

Discussed the repair to the outside resilient surfacing.

Discussed the importance of reminding parents to send an icepack or have staff ensure that food brought from home is stored appropriately.

Comments:
An unannounced Monitoring inspection was conducted today. A total of 15 children with 6 staff were present during the inspection. A total of 5 classrooms were in use and observation of all 5 classrooms was made. The children were observed transitioning to lunch. Infants were sitting at the feeding table with staff assisting with spoon feeding. Staff were observed sitting with the older children in the remaining classrooms while the children ate lunch. Handwashing procedures were observed for some children who had finished lunch. No medications onsite. Topical ointments were reviewed and parent authorizations were complete. A total of 5 children's records and 2 new staff records were reviewed. All other staff records remain current with background checks, trainings and certifications. Standard #: 8VAC20-770-60-C-2
Description: Based on staff record review, one background check was not obtained within 30 days of hire.
Evidence:
Staff #2's start date was 07/25/2022. A central registry search was requested on 07/19/2022. Results of the request was not obtained.

Plan of Correction: Contact with Office of Background Investigations will be made to inquiry about the request. The results of the background search will be forwarded to the licensing inspector within 10 days for review.

Standard #: 8VAC20-780-70
Description: Based on staff record review, one record did not contain all required information.
Evidence:
Staff #2's record did not include 2 references and documentation of completed orientation training. Staff #2 was rehired on 07/25/2022.

Plan of Correction: Ongoing any staff rehired will be considered new staff and all required documentation for new hires will be obtained.

Standard #: 8VAC20-780-245-M
Description: Based on staff record review, documentation of training did not include all requirements.
Evidence:
Staff #1 and #2's training documentation did not include the training delivery, the name of the trainer and the number of training hours.

Plan of Correction: We will revise our training documentation form to include all these required items.

Standard #: 8VAC20-780-280-H
Description: Based on observation of the center, one purse was found accessible to children.
Evidence:
Staff #2's purse was found within reach of children in an unlocked closet in the Foxhound Classroom.

Plan of Correction: The purse was relocated out of reach of children in care.

Standard #: 8VAC20-780-560-N
Description: Based on observation of the center, food was stored in an unsanitary manner.
Evidence:
A yogurt brought from home for one child was stored at room temperature in a cubby space in the Foxhound class.

Plan of Correction: All food required to be kept cold will either be stored in the refrigerator and brought from home with an ice pack.
