Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, training, and where the Site Supervisor could obtain a copy of the snack menu to post.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 09/15/2022, between the hours of 3:30 pm to 5 pm. There were 24 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records were reviewed. Staff records were reviewed on an earlier date.

Information gathered during the inspection determined that there were (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on observation, the center did not maintain a current written list of all children's allergies, sensitivities, and dietary restrictions (documented in the allergy plan required in 8VAC20-780-60 A 8).

Evidence -
1. On the date of inspection (09/15/2022), the allergy list that were observed on site was from last school year (dated 2021 - 2022).
2. This list was observed on the site board (covered to maintain confidentiality).

Plan of Correction: Will complete for this year.

Standard #: 8VAC20-780-60-A
Description: Based on review, 1 of 5 children files indicated that not all required information / documentation was contained within each child's record.

Evidence -
1. On the date of inspection (09/15/2022), the health forms (60.A.17) to include an immunization page and the documentation of a physical examination was not available for review.
2. Although this documentation is could also be located in the on-site school office, the licensed program does not have access to these files during the operational time, from 2 pm until 6 pm; Monday through Friday.

Plan of Correction: Will get from the parent of Child #1.

Standard #: 8VAC20-780-560-F
Description: Based on observation and interview, the menu for this week was not posted.

Evidence -
1. On the date of inspection, the snack menu for this week was not observed posted on the site bulletin board.
2. The Staff #1 stated (during an interview conducted on the date of inspection - 09/15/2022) that he was unaware on how to obtain this documentation, but would inquire others with such knowledge.
