An unannounced complaint inspection was conducted in response to a complaint received by the department on August 23, 2022 regarding allegations in the area of supervision. The inspection was initiated and completed on September 12, 2022. The inspector was on site from approximately 8:45 am until 10:45am. Interviews with staff were conducted, observations were made and documentation was reviewed. The preponderance of evidence gathered during the investigation did not support the allegation of non-compliance with standards or law and therefore was not valid. However violations not related to the complaint but identified during the course of the investigation were cited and can be found on the violation page. If you have any questions about this inspection, please contact the Licensing Inspector.

A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
