Comments:
An unannounced complaint inspection was conducted in response to a complaint received by the department on August 23, 2022 regarding allegations in the area of supervision. The inspection was initiated and completed on September 12, 2022. The inspector was on site from approximately 8:45 am until 10:45am. Interviews with staff were conducted, observations were made and documentation was reviewed. The preponderance of evidence gathered during the investigation did not support the allegation of non-compliance with standards or law and therefore was not valid. However violations not related to the complaint but identified during the course of the investigation were cited and can be found on the violation page. Description: Based on review of three injury reports, the facility did not ensure all required information was documented regarding children?s serious and minor injuries.

Evidence:

1. Injury report #1, dated 08/19/2022, for child #1 did not have how the parent was notified of the injury documented as required.
2. Injury report #2, dated 08/22/2022, for child #1 did not have the time the parent was notified of the injury documented as required.

Plan of Correction: Per the director: I will review the injury report form with staff to point out the importance of completing the form and make sure old forms that do not include all required information request are not being used.
