Comments:
A monitoring inspection was conducted on 9/20/2022. There were 29 children present, ranging in ages from 2 to 4 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit review.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based upon observation, the facility has not ensured that equipment is safe.
Evidence:
There was a puppy dog pull toy in the toddler classroom that had a long (1 1/2 foot ) thin piece of string used to pull the toy along the ground. The thin string is a potential entanglement hazard.

Plan of Correction: The program director responded with the following during the exit review:
The string will be removed from the pull toy. Toddler staff will be reminded to check all toys and equipment to ensure they are safe and meeting current recommendations for use.

Standard #: 8VAC20-780-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children.
Evidence:
There were three containers of hand sanitizer with warning labels on a cabinet behind a teacher's desk in the young preschool classroom.

Plan of Correction: The program director responded with the following during the exit review:
All products with warning/caution labels will be stored in areas that are locked or have child proof latches.

Standard #: 8VAC20-780-320-B
Description: Based upon observation and staff interview, the facility has not ensured that restroom handwashing sinks are equipped with disposable towels.
Evidence:
1. The children were observed washing their hands for lunch and using small cloth towels to dry their hands. Disposable towels were not available.
2. Staff 1 acknowledged that the children were using cloth towels while waiting for the paper towel dispenser to be re-filled.

Plan of Correction: The program director responded with the following during the exit review:
Paper towels were replaced during the inspection. In the future, we will ensure that maintenance replenishes the paper towels as needed.

Standard #: 8VAC20-780-430-I
Description: Based upon observation, the facility has not ensured that any personal articles used are individually assigned.
Evidence:
1. Child 2 in the toddler classroom was observed sucking on a pacifier. The pacifier was not labeled with the child's name.
2. Children in all three classrooms had individual water bottles that they had brought from home. Many of the bottles in all three rooms were not labeled to identify the child to whom the bottle belonged.

Plan of Correction: The program director responded with the following during the exit review:
If pacifiers are used, they will be labeled with the child's name.
Parents will be asked to label their child's water bottle with their child's name. If the bottles are not labeled, staff will add the names to the bottles.

Standard #: 8VAC20-780-560-J
Description: Based upon observation, the facility has not ensured that tables are sanitized by spraying with a sanitizing agent and allowing to air dry before eating.
Evidence:
The desks in the young preschool classroom where children ate lunch were sanitized using sanitizing wipes.

Plan of Correction: The program director responded with the following during the exit review:
Sanitizing solutions will be used to sanitize before eating. The surface will be sprayed and allowed to air dry as per the directions on the sanitizer.
