Comments:
A monitoring inspection was conducted on 09/21/2022. There were 58 children present and 6 staff. A tour of the facility was conducted, in addition to a record review and interviews with staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 5 children?s records along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a record review the center did not have all the required information for each enrolled child.

Evidence:
Upon review of 5 children's records, it was observed the application used to document the required information did the include the following:
a. nickname
b. work phone number and place of employment of each parent who has custody
c. Name and phone number of child's physician
d. Names of persons authorized to pick up the child,
e. Allergies and intolerance to medication,
f. Chronic physical problems and pertinent developmental information and any special accommodations needed
g. Previous child day care and schools attended by the child
h. concurrently attending schools
i. first and last dates of attendance

Plan of Correction: The children's enrollment form had recently moved to an online platform. The corporate department is looking into adding all required components to the enrollment application.

Standard #: 8VAC20-780-90--A
Description: Based on record review the center did not ensure al required written agreements were in each child's record.
Evidence:
The following required written agreements were not in the 5 children's records reviewed:
1. An authorization for emergency medical care
2. A statement that the center will notify the parent when the child becomes ill
3. A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.
