Comments:
A monitoring inspection was initiated on September 22, 2022 and concluded on September 26, 2022. There were 37 children present with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observations, the center did not ensure all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.

Evidence: During the inspection on September 22, 2022, there were two bathroom areas located inside classrooms #1 and #3 observed with areas of cracked paint and rusting throughout nearby the toilet. The metal parts were observed to be loose and rusted protruding with nails.

Plan of Correction: The center put in work orders to address areas.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence: In Classroom#3, there were two bottles of dishwashing liquid located on a shelf unlocked with the labels "keep out of reach of children" and "warnings".

Plan of Correction: The center staff immediately locked hazardous substance during the inspection.
