Reminder that visiting/covering staff members have to have their records on site. Practice emergency evacuation drills need to be conducted and documented monthly. Two shelter-in-place need to be conducted and documented yearly.

An unannounced renewal inspection was conducted with the director on August 29, 2022 beginning at 6:53 AM and ending at 7:37 AM. There were 10 school age children in the care of 2 staff members. The children were observed playing games and doing table top activities. Five children?s records and two staff records were reviewed and determined to be in compliance. The first aid kit, flashlight, and battery-operated radio were reviewed and in compliance. Two medications were reviewed and in compliance. The most recent emergency escape drill was documented as 8/26/2022. The most recent shelter in place drill was documented as 4/25/2022.

Standard #: 8VAC20-780-130-A Description: Based on a review of documentation, the facility did not ensure that all children had documentation of immunization by first day of attendance. Evidence Child A, child B, and child C did not have documentation of immunizations in their records. Plan of Correction: The facility will request documentation of immunization from the parent and place it in the children's records.
Standard #: 8VAC20-780-140-B Description: Based on a review of documentation, the facility did not ensure that all children had documentation of a physical within 30 days of attendance. Evidence Child A, child B, and child C did not have documentation of a physical in their records. Plan of Correction: Documentation of a physical will be requested from the parent and place in the children's records.
Standard #: 8VAC20-780-60-A Description: Based on a review of five children?s records, the facility did not ensure that all of the children?s records contained the required information. Evidence: Child A?s record was missing emergency contact address. Child B?s record was missing a second emergency contact. Child C?s record was missing second emergency contact?s address. Child D?s record was missing emergency contact information. Plan of Correction: The facility will request the missing documentation from the parent and the child's record will be update once it is received.
