Comments:
An unannounced monitoring inspection was conducted on 09/26/2022 from11:45 a.m. to 2:15 p.m. There were 52 children present, ranging in ages from 6 months to 5 years old, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on observation and interview, the licensee failed to ensure that the center maintained compliance with the center?s own policy regarding safe sleep.
Evidence:
1. In Classroom #2, 1 child was observed sleeping in a bouncer chair.
2. In Classroom #3, 2 children were observed sleeping in bouncer chairs.
3. Staff #6 provided the center?s safe sleep policy for review and confirmed that the center?s policy included that children should be placed for sleep ?in a supine position? and ?on a firm sleep surface?.

Plan of Correction: All infant teachers have been retrained on the sleep safe policy 9/27/22. Infants will sleep in cribs.

Standard #: 8VAC20-780-60-A
Description: Based on review of records and interview, the center failed to ensure that each child?s record. contained all required information.
Evidence:
1. Child #1?s record did not contain a work phone number for one of the parents. Staff #6 confirmed the record did not contain a work number for one for the parents.
2. Child #2?s record did not contain a work phone number for one of the parents, name of the physician or a number for the child?s physician. Staff #6 confirmed the child?s record did contain a work phone number for one of the parents, name of the physician or a number for the child?s physician.
3. Child #4?s record did not contain a work phone number for one of the parents. Staff #6 confirmed the record did not contain a work number for one for the parents.

Plan of Correction: All files will be updated with parent work numbers, doctor's name and phone
number by 10/3/22.

Standard #: 8VAC20-780-90--A
Description: Based on observation and interview, the center failed to ensure that the parent agreement was signed by the child?s first day of attendance.
Evidence:
1. Child #1?s first day of attendance was 09/26/2022 and the parent agreement was signed on 09/15/2022.
2. Staff #6 confirmed that the parent agreement for Child #1 was not signed by the child?s first day of attendance.

Plan of Correction: Parent agreement will be signed on first day of child's attendance

Standard #: 8VAC20-780-240-A
Description: Based on review of records and interview, the center failed to ensure that all staff completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
1. Staff #3?s start date was 04/18/2022.
2. The date of completion for the Virginia Department of Education-sponsored orientation course for Staff #3 was 09/20/2022.
3. Staff #6 confirmed that Staff #3 did not complete the Virginia Department of Education-sponsored orientation course within 90 days of employment.

Plan of Correction: New staff will complete the Virginia Preservice training within 90 days of their hire date.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, the center failed to ensure that hazardous substances were kept in a locked place.
Evidence:
1. In Classroom #1, Equate shaving cream was observed in the bottom shelf of an unlocked storage closet. Staff #7 confirmed the shaving cream was not in a locked place.
2. In Classroom #2, commercial air freshener, water cleaning tablets and Mr. Clean floor cleaner were observed in an unlocked lower cabinet. Commercial air freshener and commercial disinfectant spray were observed in an upper unlocked cabinet. Staff #5 confirmed that the cabinets were not locked.

Plan of Correction: Maintenance department installed lock on the cabinet with shaving cream on
9/27/22. Teachers have been reminded that all chemicals must be locked, locks must remain locked and keys may not stay in locks.

Standard #: 8VAC20-780-520-B
Description: Based on observation and interview, the center failed to ensure that sunscreen containers were labeled with the child?s names.
Evidence:
1. In Classroom #1, 11 containers of sunscreen were not labeled with a child?s name.
2. Staff #6 confirmed the sunscreens were not labeled with a child?s name.

Plan of Correction: All sunscreens were removed from classrooms on 9/26/2022. New sunscreens will be labeled with child's name.

Standard #: 8VAC20-780-530-A-1
Description: Based on observation and interview, the center failed to ensure that at least one staff in each classroom where children are present had current cardiopulmonary resuscitation (CPR) certification.
Evidence:
1. Staff #5 was observed to be the only staff member in Classroom #2.
2. Staff #5 did not have CPR certification on file.
3. Staff #6 confirmed that Staff #5 did not have CPR certification and was the only staff member in Classroom #2.

Plan of Correction: Teacher was placed in a room with a staff that has current CPR/FA on 9/26/2022. Any staff without CPR/FA will not be allowed alone in a class and CPR/FA class is scheduled for 10/1/22.

Standard #: 8VAC20-780-530-A-2
Description: Based on observation and interview, the center failed to ensure that at least one staff in each classroom where children are present had current first aid certification.
Evidence:
1. Staff #5 was observed to be the only staff member in Classroom # 2.
2. Staff #5 did not have first aid certification on file.
3. Staff #6 confirmed that Staff #5 did not have first aid certification and was the only staff member in Classroom #2.

Plan of Correction: Teacher was placed in a room with a staff that has current CPR/FA on 9/26/2022. Any staff without CPR/FA will not be allowed alone in a class and CPR/FA class is scheduled for 10/1/22.
